To support patients in their own homes to live as fully as possible with their life-limiting condition.
The Palliative Care Assistant will be the eyes and ears for the Registered Professionals who hold responsibility for the patient's care.
The Palliative Care Assistant will use excellent communication and listening skills to enhance patient's psychological wellbeing; be adept at recognising when patient's conditions change and report this to the relevant member of the specialist Multidisciplinary.
Who are we looking for?
Our specialist community team provides care for people with complex symptom management due to life limiting illnesses within the home setting and support to carers of the individual.We are looking for someone who is self motivated, caring and kind with excellent communication and listening skills who is adept at recognising when patients’ conditions change
Community experience/Palliative care experience is desirable but not essential
