Job Description

St Peter & St James Hospice specialist community team provides care for people with complex symptom management due to life limiting illnesses within the home setting and support to carers of the individual by assisting them with the support that they need.

This is an exciting opportunity to work as part of a friendly specialist multidisciplinary community team providing palliative nursing care interventions helping patients to live and die well and ensuring their carers and families can access the support that they need.

Working autonomously, the Community Registered Nurse will be working alongside the Clinical Nurse Specialists in supporting a caseload of patients with specialist palliative care and end of life care needs in the community, assessing referrals appropriateness and urgency and effectively identifying nursing best practice developments to support the clinical team.

Who are we looking for?

An opportunity has arisen for a self motivated, caring, kind and experienced registered nurse to join this well established community team. We are looking for someone who has excellent communication, assessment and prioritisation skills and understands the needs of those receiving palliative or end of life care.

As a member of the multidisciplinary team, our nurses are extremely proud of the expert, compassionate care given our patients. They actively support the Trust’s mission in the delivery of high quality care by assessing, planning, implementing and evaluating programmes of care to meet the needs of patients across all hospice services.

Community experience with clinical palliative care knowledge would be valuable, although not essential.
