Referral Process
We support anyone in our community with a life-limiting illness to live and die well, according to what matters most to them. We welcome referrals for patients from the point of diagnosis, so we can support them, their family and friends.
Referral Criteria
To make a referral, please note we accept referrals for anyone who meets all of the following criteria:
- Has a life-limiting illness
- Is over the age 18
- Has care needs that cannot be met by their usual care provider
- Lives in our catchment area
- Has a registered GP in our catchment area
