Referral Process

We support anyone in our community with a life-limiting illness to live and die well, according to what matters most to them. We welcome referrals for patients from the point of diagnosis, so we can support them, their family and friends.

Referral Criteria

We accept referrals for anyone who meets all of the following criteria:

  • Has a life-limiting illness
  • Is over the age 18
  • Has care needs that cannot be met by their usual care provider
  • Lives in our catchment area
  • Has a registered GP in our catchment area

Not in our catchment area?

If the patient does not live in our catchment area and you are unsure which hospice you should refer to, please use the Hospice UK hospice finder tool, or contact us via our contact form and we will be happy to assist you.

Self Referral 

If you meet our referral criteria, outlined above, you can access our services by completing the form below. This form is monitored Monday to Friday. If you need urgent help, please call your GP, 111 or 999, depending on the nature of your emergency.
